It is our philosophy to help you look and see your personal best with a minimum of downtime. We are dedicated to providing the highest level of patient care. This includes providing every patient with:

APPOINTMENTS:


Monday: 10:00 am – 2:00 pm
Tuesday – Friday: 9:30 am – 5:00 pm

Please provide us with as much advance notice as possible for any appointment changes. This will enable us to better accommodate other patients.

PRESCRIPTION REFILLS:


Prescription and contact lens refills are handled during office hours when we have full access to your medical records. Kindly give us at least 48 hours’ notice of prescription refills and call us before you run out of your medication. Refills are not routinely filled after hours, on holidays and weekends, or if you have not been seen for more than six months.

AFTER HOURS:


In the case of an emergency, please go to the nearest emergency room or call 911.

FEES, PAYMENT & INSURANCE:


We participate in many insurance plans. Please provide our office with completed and signed registration forms, billing information, a copy of insurance card, and all required referral and authorization forms prior to, or at the time of, each visit. Let us know if your insurance has changed, or if you have moved, so we may update your information.

If you are a new patient, we have provided a link on the left to registration forms to fill out and bring with you on your initial visit.

We collect your insurance plan’s co-pay at the time of your office visit. We accept cash, check, Discover, Mastercard or Visa payments.

For our patients who do not carry medical insurance, we require full payment at the time services are rendered by cash or credit card. All elective cosmetic procedures must be paid for in advance. If paying for an elective procedure by personal check, it must be paid in full ten (10) business days prior to the procedure.

FINANCING


We participate with Care Credit for financing. Care Credit’s financing options will depend on your credit and desired program. You may also pay for elective services on your credit card.

CANCELLATION & “NO SHOW” POLICY


Canceling or rescheduling examination appointments with less than 24 hours’ notice or “no shows” may result in a charge starting at $50.00. Canceling or rescheduling procedure appointments with less than 48 hours’ notice or “no shows” may result in a charge ranging from $200 to the full cost of the procedure.
